Temporary Disability
The Disability Resource Center (DRC) at Cal Poly provides services for students with temporary disabilities who register with our office.
The first step is to complete and return the Student Application for Temporary Services as soon as possible so that we can begin to process your request. Students must verify their disability by providing appropriate documentation from a physician or licensed professional. Download the Medical Documentation Form for the physician to complete; documentation should specify services necessary and anticipated duration of services.
Once documentation has been turned in to our office, students with temporary disabilities may be authorized for these services, as needed.
